  1. significant changes in status
    Hospital stay longer than 24 hours
    Decline in health status (becomes palliative)
    New order for Oxygen / IV

  2. Prevalence assessment should be done on admission and at least once per month on each patient.
    Appropriate surfaces to prevent pressure injury should be in place. Example air-mattress and air-cushion for wheel-chair
    Proper daily documentation on skin assessment from admission and through-out the patient's stay in the hospital
